Health Education Services

The goal of the Health Education Services department is to support clinic staff by providing education and support to MCPN patients in the following program areas:

Diabetes:

  • Individual education is provided by our diabetes educators for patients with pre-diabetes, type 1 and type 2 diabetes.
  • Diabetes group visits are provider visits in a group format with support and education components from the diabetes educator.
  • Gestational Diabetes – Education is provided by Health Corps members to women having pregnancy related diabetes.
  • Retinal Screening – a screening program to detect eye disease (retinopathy) resulting from diabetes.

Weight Loss:

  • Based on the “America on the Move” program individual education and counseling are provided by a health educator to overweight patients referred by their MCPN provider.

Smoking Cessation:

  • Education, support and encouragement are provided in individual sessions to patients by a health educator.

Cardiovascular Disease (CVD) Reduction Grant with Denver Health:

  • This community outreach program targets under and uninsured community members in Arvada, with an emphasis on reaching people of color. Cardiovascular disease screens (weight, blood pressure and cholesterol checks) are done in local businesses and organizations.

Dental Clinic Health Education:

  • Health education is provided to adult patients and their children about proper oral hygiene during toddler years. Education also promotes early dental visits (as soon as when baby teeth start coming in) to reduce incidence of cavities in baby teeth.

Other Programs and Services:

  • Por Tu Familia: In collaboration with the American Diabetes Association, an outreach program to Latinos about diabetes prevention and heart health.
  • Prenatal Nutrition Education: HealthCorps members provide nutrition education to pregnant women.
