Diabetes and Health Education Programs

The goal of the Diabetes and Health Education Programs department is to support clinic staff by providing education and support to MCPN patients in the following program areas:

Diabetes:

  • Individual education is provided by our diabetes and health educators for patients with pre-diabetes, type 1 and type 2 diabetes.
  • Diabetes group visits are provider visits in a group format with support and education components from the diabetes and health educators.
  • Gestational Diabetes – Education is provided by Health Corps members to women having pregnancy related diabetes.
  • Retinal Screening Program for patients with diabetes to detect early diabetic eye disease (retinopathy).

Overweight, High Blood Pressure, and High Cholesterol:

  • Based on the “America on the Move” program individual education and support are provided to overweight patients referred by their MCPN provider.
  • Low salt and low cholesterol dietary counseling is provided by a health educator or HealthCorps member for patients who have high blood pressure (hypertension) and high cholesterol (hyperlipidemias).

Smoking Cessation:

  • Education, support and encouragement are provided in individual sessions to patients by a health educator.
